Medical & Dental Disclaimer — Please Read First

This website is for general information only. It is not dental or medical advice, and it is not a substitute for an examination, diagnosis, or treatment by a licensed dentist or physician.

No doctor–patient relationship. Reading this Site, submitting a form, taking our online quiz, calling, texting, or scheduling a consultation does not create a doctor–patient relationship with Center for Implant Dentistry, Dr. Sambhav (Sam) Jain, or any member of our team. That relationship begins only after an in-person examination and a mutual, documented agreement to treat.

Every mouth is different. Whether dental implants — including same-day and full-arch All-on-8 ROBUST® treatment — are right for you can only be determined through a clinical examination and 3D imaging. Descriptions of procedures, timelines, and outcomes on this Site are general educational information, not a promise about your case.

Results vary. Before-and-after photographs, reviews, and testimonials reflect individual patients’ experiences. They are not a guarantee, warranty, or prediction of your outcome. All surgical and dental procedures carry risks, which your doctor will review with you before any treatment.

Never delay care because of this Site. Do not disregard or postpone professional advice because of something you read here.

1. Acceptance of These Terms

These Terms of Use (“Terms”) govern your access to and use of the website located at samedaydentalimplantcenter.com and its content, forms, and features (the “Site”), operated by Center for Implant Dentistry in Fremont, California (“we,” “us,” or “our”). Certain features may be subject to additional posted guidelines or terms, which are incorporated into these Terms by reference.

By accessing or using the Site, you agree to these Terms and to our Privacy Policy. IF YOU DO NOT AGREE WITH ALL PROVISIONS OF THESE TERMS, DO NOT ACCESS OR USE THE SITE.

2. Eligibility and Permitted Use

The Site is intended for adults age 18 and older located in the United States who are researching dental implant care for themselves or someone they assist. You may use the Site only for lawful, personal, non-commercial purposes consistent with these Terms, and you agree to comply with all applicable local, state, and federal laws in connection with your use.

3. Informational Content

We work to keep the Site’s clinical information accurate, current, and consistent with accepted practice, and content is prepared or reviewed under the direction of our doctors. Dentistry evolves, however, and information may become outdated or contain errors. We may update, correct, or remove content at any time without notice. The Site’s content is provided “as is” for education — your treatment decisions should always be made with a licensed clinician who has examined you.

In addition to the prominent disclaimer at the top of these Terms: all information on the Site regarding dental implants, full-arch rehabilitation, the All-on-8 ROBUST® protocol, same-day treatment, costs, healing, materials, success rates, or any other clinical topic is general educational information only. No doctor–patient relationship is created by your use of the Site, submission of forms, or any electronic communication.

4. Appointment Requests and Communications

  • A request is not a confirmed appointment. Submitting a consultation form or quiz tells us you’d like to talk; your appointment is confirmed only when our team confirms it with you directly.
  • Consultations and scans. Our complimentary 3D scan and consultation is an opportunity to evaluate candidacy. It does not obligate you to treatment or us to provide it.
  • By providing your phone number, you ask us to contact you about your inquiry by call or text, including via automated scheduling systems. Consent to marketing texts is not a condition of receiving care; reply STOP at any time to opt out. Message and data rates may apply.
  • Call recording. Calls to our tracking numbers may be recorded for quality and training, with notice at the start of the call. See Your Privacy Choices.
  • Do not send emergencies or sensitive details (such as Social Security numbers or full health histories) through website forms. Forms are for inquiries; clinical information is gathered securely during your visit.

5. Financing and Pricing Information

Any pricing, cost ranges, or financing information on the Site is general and illustrative. Your actual fees depend on your examination, imaging, and treatment plan, and will be quoted to you directly before treatment. Financing is offered by independent third-party lenders, subject to their own credit approval, terms, and privacy practices; we do not make credit decisions and are not responsible for lenders’ terms.

6. Intellectual Property

The Site and its contents — text, photographs, videos, graphics, smile gallery images, page designs, logos, and the marks Center for Implant Dentistry and All-on-8 ROBUST® — are owned by or licensed to us and protected by copyright, trademark, and other laws. We grant you a limited, revocable, non-exclusive license to view and print Site content for your personal, non-commercial use. You may not copy, republish, scrape, frame, sell, or create derivative works from Site content, or use our marks, without our prior written permission. Patient photographs are used with consent and may not be reused by anyone for any purpose.

7. Reviews, Testimonials, and Submissions

If you submit a review, testimonial, photograph, question, or other content to us or authorize us to display one (“Submission”), you grant us a perpetual, royalty-free, worldwide license to use, reproduce, edit for length and clarity, and display that Submission in connection with the practice, and you represent that it is truthful, your own, and that you are at least 18. We may decline or remove any Submission at our discretion. Please do not include other people’s personal information in a Submission.

Nothing in this section limits your rights under our Privacy Policy or, for patients, under HIPAA — clinical photographs and patient stories are published only with documented patient authorization.

8. Prohibited Conduct

You agree not to:

  • use the Site in any way that violates law or these Terms;
  • submit false, misleading, or impersonating information through forms or calls;
  • interfere with the Site’s operation or security, probe or test vulnerabilities without authorization, or introduce malicious code;
  • scrape, harvest, or collect information about the Site or its users, including by automated means or for training automated systems, without written permission;
  • obtain or attempt to obtain materials or information through means not intentionally made available; or
  • use the Site to advertise or solicit for any other product or service.

We may suspend or terminate access for conduct that violates these Terms.

10. Privacy

11. Disclaimer of Warranties

TO THE FULLEST EXTENT PERMITTED BY LAW, THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, NON-INFRINGEMENT, AND UNINTERRUPTED OR ERROR-FREE OPERATION. THIS SECTION APPLIES TO THE WEBSITE AND ITS INFORMATION ONLY — IT DOES NOT LIMIT OR DISCLAIM ANY PROFESSIONAL DUTY WE OWE PATIENTS IN CONNECTION WITH ACTUAL CLINICAL CARE.

12. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, CENTER FOR IMPLANT DENTISTRY, ITS OWNERS, DOCTORS, EMPLOYEES, AND AGENTS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF DATA, ARISING FROM OR RELATED TO YOUR USE OF (OR INABILITY TO USE) THE SITE. OUR TOTAL AGGREGATE LIABILITY FOR ALL CLAIMS RELATING TO THE SITE WILL NOT EXCEED ONE HUNDRED U.S. DOLLARS ($100).

THIS LIMITATION APPLIES ONLY TO CLAIMS ARISING FROM THE WEBSITE ITSELF. NOTHING IN THESE TERMS LIMITS LIABILITY FOR PROFESSIONAL DENTAL SERVICES ACTUALLY RENDERED TO A PATIENT, OR ANY LIABILITY THAT CANNOT BE LIMITED UNDER CALIFORNIA LAW, INCLUDING UNDER CALIFORNIA CIVIL CODE §1668. SOME JURISDICTIONS DO NOT ALLOW CERTAIN LIMITATIONS, SO PORTIONS OF THIS SECTION MAY NOT APPLY TO YOU.

13. Indemnification

You agree to indemnify and hold harmless Center for Implant Dentistry and its owners, doctors, employees, and agents from claims, damages, and reasonable attorneys’ fees arising out of your violation of these Terms, your Submissions, or your misuse of the Site. This section does not apply to claims arising from clinical care you receive as a patient.

14. Governing Law and Dispute Resolution

A. Governing Law and Venue. These Terms and any dispute arising from the Site are governed by the laws of the State of California, without regard to conflict-of-law rules. You and we agree to the exclusive jurisdiction and venue of the state and federal courts located in Alameda County, California, for any Site-related dispute, and each party waives objections to that venue. We may seek injunctive relief in any court of competent jurisdiction to protect our intellectual property or confidential information.

B. Contact Us First. If a dispute arises, please email us at info@bayareaimplantdentistry.com so we can try to resolve it informally.

C. Alternative Dispute Resolution. For any claim where the total amount sought is less than $10,000 USD, the party requesting relief may elect binding non-appearance-based arbitration administered by a nationally recognized provider. The arbitration may proceed by telephone, online, or written submissions. Any judgment on the award may be entered in any court of competent jurisdiction.

15. General Terms

  • Term and termination. These Terms remain in effect while you use the Site. We may suspend or terminate access at any time for violations of these Terms; upon termination, your right to use the Site ceases immediately.
  • Changes. We may revise these Terms by posting an updated version with a new “Last Updated” date; material changes will be reasonably highlighted. Continued use of the Site after changes take effect constitutes acceptance.
  • Severability. If any provision is found unenforceable, the remainder stays in effect, and the provision will be enforced to the maximum extent permitted.
  • No waiver. Our failure to enforce a provision is not a waiver of it.
  • Entire agreement. These Terms, together with the policies incorporated by reference, are the entire agreement between you and us regarding the Site. They do not modify any treatment, financial, or consent agreement you sign as a patient.
  • Assignment. You may not assign these Terms; we may assign them in connection with a practice reorganization or sale, consistent with our Privacy Policy.
