Your Choices

Depending on where you live, you may have the following options regarding your personal information collected by Center for Implant Dentistry (“CID,” “we,” “our,” or “us”).

  • Access. Request access to your personal data or confirmation that we have data about you. In some cases, you may request a portable, machine-readable copy.
  • List of Third-Party Recipients. Request a list of specific third parties to which we have disclosed personal information. Some laws also allow requesting the categories of third parties; see our Privacy Policy (How We Share Information) for categories we share with.
  • Change (Correction). If you cannot update certain information yourself, ask us to correct inaccurate or incomplete data using the contact methods below. We may retain historical records where permitted by law.
  • Deletion. Ask us to delete your personal data. If required by law, we will honor your request, though we may retain data where necessary to comply with legal obligations, resolve disputes, enforce agreements, or for other legitimate business purposes.
  • Opt-out of Targeted Advertising. Manage online tracking-based advertising (e.g., cookies) via the Your Privacy Choices control (if available) or your browser settings. Preferences may be browser/device-specific and may reset if you clear cookies. Where required by law, we recognize Global Privacy Control (GPC) signals for that browser. We do not respond to “Do Not Track” signals. If a conflict exists between a GPC signal and a manual choice you set on our Site for targeted advertising, we will honor your manual choice.
  • Promotional Emails. Unsubscribe using the link in our emails. We may still send service-related messages.
  • Promotional Mailings. If you prefer not to receive postal mail from us, email us with “NO SNAIL MAIL” in the subject line and include your name and mailing address so we can locate your record. You may receive mailers prepared prior to processing your request.

Not all rights are absolute; we may limit or deny a request where permitted or required by law. We will not discriminate against you for exercising privacy rights.

Submitting Requests

Submit requests via the Your Privacy Choices control (if available) or by using the contact information below. If you disagree with a decision, you may appeal by contacting us with the subject line “Appeal.”

We verify identity for most requests (e.g., by phone or email). Depending on the request, we may ask for your name and contact details and may request a signed declaration. You may designate an authorized agent where allowed by law; authorized agents should provide documentation showing authority to act on your behalf.

How We Protect and Retain Data

No method of internet transmission or electronic storage is fully secure, but we implement reasonable administrative, technical, and physical safeguards. If we are required to notify you of a security incident, we will do so consistent with applicable law.

If our Site allows account creation, you are responsible for choosing a unique, complex password and keeping it confidential. You are responsible for activity occurring under your credentials. Notify us immediately of unauthorized use of your account.

We keep personal data only as long as necessary to fulfill the purposes in our Privacy Policy unless a longer period is required or permitted by law (e.g., legal, accounting, or reporting obligations). We consider the amount, nature, and sensitivity of the data, the potential risk of harm, the purposes for which we collected it and whether those purposes can be met by other means, and applicable legal requirements.

Changes To This Privacy Notice

We may update this Notice. New versions will be posted on our Site. If changes are material, the Notice in effect when you submitted data will generally govern that data unless you consent to the new Notice. See the Effective and Last Updated dates at the top of this page.

California “Shine the Light”

To request information about categories of personal data (if any) we share with third parties or affiliates for their direct marketing, contact us using the details above and include “Shine the Light Request” in the subject line.

California Sensitive Information Disclosure

We may collect sensitive personal information (as defined by California law)—for example: government-issued IDs for identity verification in limited contexts; account credentials when required to access an account; health-related data associated with dental care; and precise geolocation where you choose to share it for map or location features. We use this data only for permitted purposes (e.g., to provide services, process transactions, comply with law, manage our business, prevent fraud, or with your direction/consent). We do not “sell” or “share” sensitive personal information for cross-context behavioral advertising. If we transfer such data to third parties at your direction, those parties will use it as described in their privacy notices.
